INSURANCE VERIFICATION

Insurance can only be used for Massage Therapy, Manual Lymphatic Drainage (regular and post surgical), Scar Release Therapy and Vagal Nerve Stimulation.

How to Use Your Insurance:

1. Click the link and input your insurance information on the Holbie website.

2. Holbie will verify your insurance and contact Melissa (Body in Balance) within 2 to 3 business days.

3. After your insurance is verified, Melissa will contact YOU to schedule an appointment.

4. If there is a co-pay to be paid, it will be collected by Melissa at the time of your appointment.

IMPORTANT INFORMATION:

You cannot book an appointment expecting to use insurance without following the steps above.

At this time, Medicare and Medicaid are NOT accepted

PRICING: Body in Balance Website prices reflect discounted cash payments and they are not the same as the negotiated/contracted insurance rates.

Your insurance company will be billed at the negotiated/contract rates listed below:

Massage Therapy - $45 billed in 15 minute increments

Manual Therapy - $75 billed in 15 minute increments

Neuromuscular Therapy - $45 billed in 15 minute increments
